CRM Add Broker Request Form
- Before submitting this form, please check CRM to see if the broker exists in CRM already.
- If the broker is not in CRM and you want them added to CRM, please complete the form below.
- If the broker already exists in the CRM, please be sure to indicate that you are updating an existing record and fill out the fields that need to be revised.
- Upon submission the request will be sent to the Marketing Department, who will add the broker as a “Prospect” unless instructed otherwise.
- You and your sales manager will get a notification when complete.
- Upon completion, please update the CRM record with any other additional information you have. The fields below are the minimum requirements to create a new broker.
